Due to the prevalence of chronic, non-healing wounds in patients with diabetes mellitus, the use of effective treatment methods is especially important for them. Therefore, the search for new methods of anesthesia and treatment that enhance the effectiveness of therapy for diabetic foot syndrome remains a priority area of modern medicine. The aim of the study was to compare the parameters of macro- and microcirculation, as well as clinical and laboratory parameters in elderly patients with diabetic foot syndrome with a single conduction anesthesia of the sciatic nerve followed by the use of a nitroglycerin patch and without it. The study included 40 elderly patients (60-75 years old) with Wagner stage II–III diabetic foot, as well as with combined forms (stages II–IV). There were 21 men (52.5%) and 19 women (47.5%) among them. The patients were divided into two groups of 20 people: in the first group, a single conduction anesthesia of the sciatic nerve was used without the subsequent use of a nitroglycerin patch, in the second – with its application. The inclusion criteria were the presence of type 2 diabetes mellitus complicated by Wagner stage II–IV diabetic foot. The exclusion criteria were severe cardiovascular disorders, septic condition, significant cognitive impairment, excluding contact with the patient. The studies were conducted at 3 stages before surgery, on the 10th and 15th days of the postoperative period. Clinical and laboratory parameters, as well as macro- and microcirculation parameters, were evaluated. Studies have shown that in patients with diabetic foot syndrome, when using a single conduction anesthesia of the sciatic nerve in combination with a nitroglycerin patch, there was a more pronounced improvement in clinical and laboratory parameters, macro- and microcirculation parameters compared with the group without the use of this patch. The use of a nitroglycerin patch in combination with a single conduction anesthesia of the sciatic nerve helps to accelerate wound healing in elderly patients with diabetic foot syndrome.
