The modern approach to treating periodontal diseases focuses on controlling periodontal pathogen levels and modulating the immune response. The aim of this study was to qualitatively assess periodontal pathogens in periodontal pockets and measure sIgA concentration in the oral fluid of patients with generalized periodontitis (severity stages I-II) throughout the treatment process. A total of 90 patients with I-II severity of chronic generalized periodontitis (CGP) were included in the study. Group 1 received a treatment complex including the antihomotoxic drug Traumeel C, while Group 2 received a complex with the nano-based preparation Fullerene C60. The presence of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Prevotella intermedia, Tannerella forsythia, and Treponema denticola was determined via PCR at baseline and in 3, 6, and 12 months of treatment. sIgA levels were measured using an enzyme-linked immunosorbent assay. Before treatment, Tannerella forsythia was the most prevalent pathogen, observed as a mono-infection and in associations in 57.8% and 55.6% of cases, respectively. Porphyromonas gingivalis was detected in 44% (mono-infection) and 40% (associations) of cases; Prevotella intermedia in 42% and 36%; and Treponema denticola in 40% and 37.8%. After 5 days of treatment, 2.75.6% of Group 2 patients showed no detectable periodontal pathogens. Prevotella intermedia, Aggregatibacter actinomycetemcomitans, and Tannerella forsythia were found only as monocultures, in 4.4%, 6.7%, and 6.7% of patients, respectively. After 3 months, the percentage of patients without detectable periodontal pathogens remained stable at 46.7% in Group 1 and 75.6% in Group 2. The baseline sIgA levels in both groups were initially low. Following 3 months of treatment, sIgA concentration increased, with Group 2 patients exhibiting a 27% higher sIgA concentration on average. After 6 months, the spectrum of periodontal pathogens in Group 1 returned to baseline levels. In Group 2, 24.4% of patients continued to show no detectable periodontal pathogens at both the 6-month and 1-year marks. To achieve a stable outcome, it is recommended to re-administer the treatment complex with the Fullerene C60 nanopreparation between 9 and 12 months.
