Early childhood caries is one of the major problems in pediatric dentistry. Timely diagnosis and selection of appropriate treatment strategy are important for preserving primary teeth and preventing complications and disturbances in dentofacial development. The level of parental compliance may significantly affect the possibility of performing the necessary diagnostic and treatment procedures. The aim of the study is to present a clinical case of comprehensive treatment of a patient with early childhood caries and to demonstrate the impact of low parental compliance on the diagnostic algorithm and choice of treatment strategy. A 5,5-year-old child with multiple carious lesions of the primary teeth was examined and treated. Oral hygiene status, caries intencity, occlusal relationships and the condition of oral mucosae were assessed. Age-appropriate caries indices and Fedorov-Volodkina hygiene index and Angle’s classification were used. The level of parental compliance was assessed according to clinical criteria. The child was diagnosed with early childhood caries, accompanied by chronic forms of periodontitis affecting most primary teeth and an exacerbation of chronic periodontitis of tooth 5.1. The mother refused radiographic examination which limited diagnostic possibilities, while refusal of oral rehabilitation under sedation resulted in a change of the planned treatment strategy. Staged surgical oral rehabilitation under local anesthesia was performed during four visits. Following extraction of teeth with unfavorable prognosis and their roots, the child was referred to an orthodontist for further treatment. Timely elimination of odontogenic infection foci and assessment of consequences of premature loss of primary teeth are important components of comprehencive dental care. A favorable treatment outcome is possible only with effective cooperation between the pediatric dentist and the parents. Low parental compliance may complicate the diagnostic process, influence the choice of treatment strategy and delay the timely provision of dental care to children.
