Vitamin D deficiency and insufficiency are associated with disturbances in carbohydrate metabolism and the development of insulin resistance. At the same time, the role of combined vitamin D and iron supplementation in correcting metabolic disorders requires further investigation. The aim is to evaluate the effects of vitamin D supplementation alone and in combination with iron on carbohydrate metabolism parameters and insulin resistance in women with vitamin D deficiency or insufficiency. An analysis was conducted of changes in fasting glucose, insulin levels, and the HOMA-IR index before and after intervention in groups receiving vitamin D monotherapy supplementation and combined vitamin D plus iron supplementation. Both supplementation strategies resulted in statistically significant improvements in metabolic parameters. In the vitamin D monotherapy group, a more pronounced reduction in fasting glucose levels was observed, indicating a positive effect of vitamin D on glycemic control. At the same time, combined vitamin D and iron supplementationwas associated with a more effective improvement in insulin sensitivity. Specifically, the HOMA-IR index in the combined supplementation group decreased from 1.56 to 1.13 (−27.6%, p<0.001), whereas in the vitamin D group it decreased from 2.08 to 1.40 (−32.7%, p<0.001). Despite the greater absolute change in the monotherapy group, the final insulin and HOMA-IR values remained higher compared with those in the combined intervention group. The obtained results indicate that vitamin D is an important factor in glycemic regulation, while iron may potentiate its effects on insulin sensitivity mechanisms. Combined vitamin D and iron supplementation may be considered a promising nutritional strategy for the correction of insulin resistance and the prevention of metabolic disorders.
