EFFECTIVENESS OF HIGH-INTENSITY TRAINING IN PATIENTS WITH VISUOSPATIAL NEGLECT AFTER ACUTE STROKE

Acute stroke often leads to persistent limitations in mobility, reduced independence, and the need for prolonged rehabilitation. Patients with visuospatial neglect require particular attention, since impaired attention to part of space disrupts orientation during walking, balance control, and the safe performance of functional tasks. In this category of patients, recovery of mobility should combine motor training with continuous engagement of spatial scanning, correction of body position, and adaptation of movement to changes in the external environment. The aim of the study was to determine how early high-intensity training (HIT) affects gait speed, physical endurance assessed by the 2-Minute Walk Test, and balance in patients with visuospatial neglect after acute stroke. Forty patients with visuospatial neglect after acute stroke participated in the study. The patients were divided into two groups: Group I received high-intensity training (HIT, n=20), and Group II received standard physical therapy (PT, n=20). Functional status was assessed before the rehabilitation course and again after 14 days of training. The 10-Meter Walk Test (10MWT), 2-Minute Walk Test (2MWT), and Berg Balance Scale (BBS) were used for the analysis. Reassessment showed statistically significant improvement in functional outcomes in both groups (p<0.05). In the HIT group, gait speed on the 10MWT increased from 0.45±0.15 m/s to 0.65±0.20 m/s, whereas in the standard PT group it increased from 0.44±0.17 m/s to 0.53±0.20 m/s. The 2MWT distance increased from 46.9±17.3 m to 74.6±25.2 m in the HIT group and from 45.1±22.1 m to 56.6±24.4 m in the standard PT group. BBS scores increased from 36.1±4.9 to 46.4±4.3 points in the HIT group and from 37.7±6.2 to 44.4±6.7 points in the standard PT group. The obtained results indicate the potential effectiveness of HIT as a component of rehabilitation for patients with visuospatial neglect, provided that exercise intensity, safety, and fall risk are carefully monitored.
