THE IMPACT OF STRUCTURAL IMPAIRMENTS ON ACTIVITIES OF DAILY LIVING IN PATIENTS WITH POSTTRAUMATIC LOWER LIMB JOINT CONTRACTURES

Annotation:
The scientific substantiation of comprehensive rehabilitation programs for patients with the consequences of severe combat and domestic injuries, complicated by the formation of persistent joint contractures, is highly relevant. The aim of the study is to increase the effectiveness of physical therapy for patients with posttraumatic myogenic, arthrogenic, and combined contractures of the lower limb joints by determining the impact of structural impairments using the Lower Extremity Functional Scale (LEFS) and evaluating the efficacy of their targeted remodeling using the “Omego Plus” robotic system. The study is based on data from a randomized, parallelgroup, controlled trial involving 180 patients aged 20 to 56 years. The participants were divided into three groups of 60 individuals each, depending on the predominant type of contracture (myogenic, arthrogenic, and combined), and within the groups, they were randomized into main (MG) and control (CG) subgroups. The MG patients received standard physical therapy and training on the “Omego Plus” robotic system, whereas the CG received only standard therapy. The assessment was conducted using the LEFS questionnaire at baseline, after 6 weeks, and after 3 months. The LEFS analysis results at baseline indicated pronounced limitations in daily activities across all groups (Me 36.0– 53.0 points). The dynamics of the indicators after 3 months (Visit 3) demonstrated the superiority of using the «Omego Plus» robotic system in patients with arthrogenic and combined contractures. Specifically, in the subgroup with arthrogenic contractures, the increase in functionality was 22.5 points (Me 64.5) compared to 16.5 points (Me 55.5) in the control group (p<0.000005). In cases of combined contractures, the results in the main group were also higher: an increase of 19.0 points (Me 55.0) versus 14.5 points (Me 51.0) in the control group (p=0.000032). At the same time, in patients with myogenic contractures, the increase in functionality was comparable in both groups (19.5 points in the MG versus 18.5 points in the CG, p=0.786). The obtained data confirm that the restoration of biomechanical characteristics directly correlates with the ability to perform daily tasks, and the use of the “Omego Plus” system provides a significant increase in functional independence, particularly in patients with complex structural impairments.
