MYCOLOGICAL CHARACTERISTICS OF NAIL PLATE LESIONS IN PATIENTS WITH PLAQUE PSORIASIS

Psoriatic onychodystrophy (nail psoriasis) affects 80–90% of patients with psoriasis and is even more common in psoriatic arthritis. The disease destroys the nail apparatus (matrix, bed, hyponychium) due to inflammation, and its symptoms are very diverse, which often complicates timely diagnosis and assessment of the severity of nail plate damage. The main study group consisted of 94 patients with nail psoriasis (56.4% men, 43.6% women; median age – 64.5 years). The control group was formed by 29 healthy individuals (55.2% men, 44.8% women; median age – 64.0 years). The groups were comparable in age and gender (p>0.05). Most patients had a combination of several clinical signs of nail plate changes, and some patients had three or more manifestations simultaneously. Such polymorphism of the clinical picture is important in view of the need for differential diagnosis of psoriatic nail lesions with fungal and bacterial pathology. In terms of the severity of pathological changes, patients with moderate nail plate lesions prevailed – 55 (58.5%). Significantly pronounced changes were found in 26 (27.7%) patients, minimal – in 13 (13.8%). The results of the mycological study showed that in patients with vulgar psoriasis and nail plate lesions, the fungal flora was characterized by species diversity, with dermatophytes and fungi of the genus Candida spp. being of greatest importance. At the same time, the single isolation of non-dermatophyte mold fungi required careful clinical and laboratory interpretation and was not a sufficient basis for the diagnosis of onychomycosis. Antimycotic susceptibility testing was performed on 34 fungal isolates isolated from 28 patients (29.8%) with confirmed onychomycosis. The number of isolates exceeded the number of patients, since in some cases several fungal agents were isolated from one patient. This approach allowed us to assess the antifungal profile of the mycotic flora itself. In turn, establishing the etiological role of the identified microorganisms is of fundamental importance for the subsequent selection of rational antifungal therapy.
