Dental caries remains one of the most common chronic human diseases and an important issue in preventive dentistry. In modern cariology, it is considered a dynamic biofilm-associated process, the development of which depends on the interaction between dental biofilm, the frequency of fermentable carbohydrate intake, acid exposure, enamel resistance, and the protective properties of oral fluid. This problem is particularly relevant in young people, because dietary and oral hygiene habits are formed during this period, the consumption of sweet drinks and frequent snacks may increase, and preventive dental visits often become irregular. The aim of the study was to systematize current scientific data on the role of oral fluid, oxidative stress, and quercetin in personalized dental caries prevention in young people. A literature review covering 2015–2026 was conducted using PubMed, Scopus, Cochrane Library, Google Scholar, and electronic archives of Ukrainian scientific dental journals. Thirty-three sources were included in the final thematic analysis. The review showed that the evidence-based foundation of dental caries prevention includes dental biofilm control, regular use of fluorides, reduction in the frequency of free sugar intake, remineralization therapy for initial enamel lesions, and fissure sealing when indicated. Oral fluid may be a practical material for assessing local changes in the oral cavity; however, its parameters are most valuable when interpreted together with clinical, behavioral, and microbiological data. Evidence on oxidative stress in dental caries remains heterogeneous because studies differ in design, sampling methods, patient age, and selected biochemical markers. Quercetin has experimentally supported antioxidant, anti-inflammatory, and antibiofilm properties; however, available clinical studies have not yet proven its ability to reduce caries increment or stabilize initial carious lesions. This review indicates that personalized dental caries prevention in young people should combine evidence-based local methods with individual risk assessment and evaluation of oral fluid properties. Oxidative stress markers and quercetin may be promising directions for further research, but at the current stage they do not replace fluorides, dental biofilm control, and correction of dietary risk factors.
