IMPACT OF PROLONGED WAR-RELATED STRESS ON QUALITY-OF-LIFE INDICATORS IN PATIENTS WITH CHRONIC HEART FAILURE

Prolonged war-related stress imposes an additional burden on patients with chronic heart failure, intensifying the effect of the underlying disease on physical condition, emotional well-being, and capacity for daily activity. The combination of heart failure progression with sustained psychoemotional strain, social instability, and limited access to medical care may result in adverse changes in quality of life. Identification of the patterns underlying these changes is necessary for the improvement of approaches to comprehensive patient support under crisis conditions. The aim of the study was determination of the patterns of dynamics of quality-of-life indicators in patients with chronic heart failure under the influence of prolonged war-related stress, based on an analysis of findings from original clinical studies. A systematic review was conducted in accordance with the PRISMA 2020 guidelines. The analysis included original clinical studies involving adult patients with heart failure in which quality of life, functional status, psychological characteristics, and factors potentially affecting disease course were assessed. Reviews, studies lacking clinical assessment of quality of life, and works not corresponding to the defined subject matter were excluded. The search for scientific sources was conducted in the international databases PubMed, Scopus, Web of Science, and other available information resources, using key terms related to heart failure, quality of life, psychoemotional stress, and war-related factors. Selection of publications, extraction of the required data, assessment of the risk of systematic bias, and formation of groups for synthesis were performed according to predefined criteria. Summarization of the obtained information was carried out using descriptive and comparative analysis, taking into account the design characteristics of the included studies. It was established that the quality of life of patients with chronic heart failure is shaped by the interaction of clinical manifestations of the disease, psychological state, and social environment. Deterioration of functional capacity, increased symptom severity, and anxiety and depressive manifestations are associated with a decline in the physical and emotional components of quality of life. Prolonged war-related stress intensifies these adverse changes through heightened uncertainty, disruption of social stability, and reduced access to comprehensive medical and psychological support. At the same time, preservation of social connections, adaptive coping strategies, and comprehensive treatment contribute to the maintenance of patients’ functional status. The dynamics of quality-of-life indicators in chronic heart failure under conditions of prolonged war-related stress are multifactorial in nature and depend not only on the severity of the cardiac disease but also on the influence of psychosocial factors. Regular monitoring of quality of life, psychoemotional state, and social factors is warranted for timely detection of negative dynamics and for adjustment of treatment and rehabilitation measures.
