Annotation:

Metabolic syndrome is a condition with the constant presence of pro-inflammatory mediators. This creates a “permanent readiness” of tissues for hyperreaction. As a result, there is an increase in cytokines; vasoconstriction and hypoxia occur; osteoblast differentiation slows down; osteoclast activity increases, which leads to osteogenesis disorders. This causes impaired primary stability of dental implants; slow regeneration of bone tissue in the area of surgical intervention; increase and resorption in the first year after implantation. Also, against the background of this syndrome, significant microvascular lesions occur, resulting in: impaired blood supply to the area around the implant; reduced tissue metabolism; reduced number of fibroblasts; slowed healing and remodeling of bone tissue. There is also a negative impact on the mucous membrane: it is swollen, hyperemic, easily damaged, quickly forms deep peri-implant pockets, which leads to such a complication as mucositis. This creates a high risk of mucositis transitioning to a more severe form with the transition to the bone structures around the implant zone – peri-implantitis. In order to minimize the risk of peri-implantitis and inflammatory complications, it is recommended to give preference to cementless (screw or friction) bridge prostheses, in particular those made using galvanic technology (Au 99.9%), which provides: maximum passivity of fit, absence of cement residues as a key trigger of inflammation, optimal distribution of chewing load, increased long-term stability of peri-implant tissues. In addition to the above, in patients with metabolic syndrome, it is advisable to use a protocol of delayed functional loading, which provides more predictable osseointegration in conditions of impaired bone metabolism and chronic systemic inflammation. Thus, the combination of preoperative preparation, installation protocol and delayed loading using galvanic cementless structures is the most biologically justified and clinically predictable rehabilitation strategy for patients with metabolic syndrome.
