Unjustified use of antibiotics in viral acute rhinosinusitis (ARS) contributes to the development of antimicrobial resistance and, in the long term, leads to increased treatment costs. The application of evidence-based medicine approaches allows avoidance of prescribing ineffective medications, reduces the risk of adverse reactions, and opti mizes pharmacotherapy costs. The aim of the study was to perform an integrated ABC/VEN analysis of antibacterial drug use in order to optimize the treatment of acute rhinosinusitis and provide an economic justification for the ra tional use of medications in inpatient settings. The dynamic ABC/VEN analysis made it possible to assess changes in the structure of prescriptions and financial expenditures on ARS pharmacotherapy. It was established that in 2025, expenditures on pharmacological management increased 1.5-fold compared to 2020, despite a reduction in the total number of medications included in the formulary. At the same time, total expenditures on antimicrobial agents increased 1.6-fold. Expenditures on protected aminopenicillins and third-generation cephalosporins remained nearly unchanged, whereas second-generation cephalosporins were not used. Analysis of expenditure structure according to VEN categories demonstrated an increase in the proportion of vital drugs (category V) within group A from 58.8% in 2020 to 79.6% in 2025. In contrast, expenditures on non-essential drugs (category N) were almost completely eliminated. The obtained results indicate a transition toward more rational pharmacotherapy and more efficient use of budgetary resources. Cost optimization can be achieved by excluding irrational drugs from group A and increasing the proportion of affordable domestic medications without compromising treatment quality.
