The condition of the oral cavity of infants determines not only the dental, but also the general health of the child, since disturbances in the formation of the oral microbiome at an early age are associated with complications in older age. Early childhood caries, candidiasis, infectious and inflammatory diseases of the mucous membrane remain among the most common pathologies of the child population, which necessitates prevention even before the eruption of the first teeth. Aim is analysis of modern European protocols for oral care in infants, assessment of the evidence base on the use of breast milk and colostrum, determination of the role of medical professionals and parents in this process. It has been established that oral care of infants is an integral part of the prevention of systemic and dental diseases and is associated with a decrease in the frequency of fungal lesions, secondary infections and early childhood caries. The oral microbiome is dynamically formed from birth under the influence of the type of delivery, feeding and environment. The effectiveness of care is determined by the coordinated interaction of pediatricians, neonatologists, dentists and parents with a clear division of responsibilities. The potential benefits of colostrum and breast milk for maintaining the mucosa and forming a physiological microbiota have been identified, however, the evidence base remains limited, which requires further randomized studies. A standardized care algorithm is presented in accordance with the EFONI recommendations. Early oral care of infants is the first stage of continuous prevention of dental diseases. The most effective results are achieved under conditions of interdisciplinary partnership between medical staff and parents, and the integration of European standards requires further standardization of protocols and multicenter studies.
