Annotation:

The article examines contemporary approaches to cardiac rehabilitation in the context of current challenges, in accordance with the ESC 2026 guidelines and the WHO Global Initiative “Rehabilitation 2030”. The relevance of developing cardiac rehabilitation is determined by a significant increase in cardiovascular diseases: more than 60 million potential years of life are lost among European residents due to cardiovascular pathology. There is a distinct “gap” between actual life expectancy and healthy life expectancy (healthspan-lifespan gap), which has expanded to 9.6 years over the last two decades. The aim of the article was to analyze contemporary methods and approaches to cardiac rehabilitation that enhance its efficacy, as well as to consider the viability of applying a comprehensive, multi-component approach to improve the long-term prognosis of the patient. In accordance with the updated ESC 2026 guidelines, cardiac rehabilitation is shifting away from the traditional “exercise-based CR” model, as it no longer meets the current needs of patients and demands a comprehensive approach and long-term control of cardiovascular risk. The key components of modern cardiac rehabilitation include individualized exercise prescription, physical activity counseling, optimization of medical therapy, nutritional support and body composition control, smoking cessation, psychosocial support, patient education, and the development of self-management skills. An important direction of modern cardiac rehabilitation is digital and hybrid care. ESC 2026 considers digitally enhanced home-based cardiac rehabilitation and hybrid telerehabilitation as additional delivery models for appropriately selected patients. Psychosocial and behavioral support is an integral component of cardiac rehabilitation. Current guidelines provide for the systematic assessment of patients’ psychosocial status, including the screening for anxiety and depression symptoms, followed by individualized intervention as needed.
