Annotation:

In dental surgical practice, many patients suffer from purulent inflammation of the maxillofacial area. In order to conduct effective conservative treatment, a dentist must have in-depth knowledge of the pharmacology of modern antibacterial agents and scientifically sound rules for their use. The purpose is to study modern antibacterial drugs' pharmacological properties and analyse the indications for their use in surgical practice. An analysis of literature sources was carried out. The results obtained indicate a limited choice when dentists prescribe antibacterial agents. The analysis of pharmacological properties and spectra of antimicrobial activity of known antibacterial drugs shows that beta-lactam drugs of the cephalosporin and carbapenem group are preferable. Cephalosporin and carbapenems are particularly relevant in treating severe polymicrobial infections and complicated purulent inflammation. In case of severe infections caused by multidrug-resistant gram-positive microorganisms, ketolides and streptogramins are recommended. In case of ineffectiveness of other antibacterial drugs, glycopeptides also have backup value. Considering the complexity and dynamism of the group of antibacterial drugs, a practising dental surgeon should rely on scientifically based recommendations for their prescription and follow the algorithm of patient management developed by scientists: to start treatment early; based on clinical and laboratory data, conduct empirical therapy with broad-spectrum antibacterial drugs, considering the most likely pathogens; when determining the pathogen, if necessary, replace the drug used for empirical treatment with another one to which the microorganism is sensitive; consider the feasibility of prescribing combined ABT; use microbiological control and monitoring of pathogens during long-term treatment.
