КЛІНІЧНІ ВІДМІННОСТІ ГАСТРОІНТЕСТИНАЛЬНИХ ПОРУШЕНЬ У ХВОРИХ НА ХРОНІЧНЕ ОБСТРУКТИВНЕ ЗАХВОРЮВАННЯ ЛЕГЕНЬ ТА БРОНХІАЛЬНУ АСТМУ

Анотація:

У статті наведено порівняння частоти, структури та клінічних відмінностей гастроінтестинальних (ГІ) порушень, зумовлених гастроезофагеальною рефлюксною хворобою (ГЕРХ) у хворих на хронічне обструктивне захворювання легень (ХОЗЛ) та бронхіальну астму (БА), та проаналізовано їх вплив на перебіг основного захворювання. Виявлено, що у хворих на БА гастроінтестинальні порушення зустрічаються достовірно частіше порівняно з хворими на ХОЗЛ (60% проти 45%, p<0,05) та є типовими для стравохідного рефлюксу: типова печія, кисла регургітація. У хворих на ХОЗЛ частіше зареєстровано атипові гастроінтестинальні симптоми: хронічний кашель без печії, відчуття переповнення шлунка, відрижку повітрям, захриплість. Встановлено, що наявність гастроінтестинальних симптомів у обох групах хворих асоціюється з погіршенням перебігу основного захворювання, проте цей зв’язок більш виражений у пацієнтів з ХОЗЛ. У групі ХОЗЛ наявність ГІ симптомів асоціювалася зі значним підвищенням ризику загострень (OR 2,1-5,3; p<0,01). У групі БА спостерігався помірний підвищений ризик нічних нападів (OR 1,3-1,5; p<0,05). Отримані результати підкреслюють важливість скринінгу гастроінтестинальних симптомів у пацієнтів обох груп для оптимізації лікування та профілактики загострень.
