Over the past two years, the number of mine and explosive injury victims in Ukraine has been increasing. The main mass share of landmine-explosive wounds, namely about 70%, consists of damage to soft tissues of various degrees of contamination and various areas. The aim of the study was to improve the quality of life of patients and reduce their periods of disability by developing a step-by-step model for the treatment of mine-explosive injuries with massive soft tissue damage. The results of the surgical treatment of 103 victims among the civilian population with landmine-explosive injuries and large-area soft tissue wounds using a stepwise treatment model were analysed. The first stage was debridement of wounds with the removal of foreign bodies (if they were present), necroctomy. In the second stage, VAC therapy was used to clean the wounds and reduce their area. The third stage was free autodermoplasty with a perforated skin flap. It has been established that a phased approach to the treatment of blast injuries with massive soft tissue damage provides good treatment outcomes, improving surgical outcomes for such patients and reducing their period of incapacity for work, making it economically viable.
