Annotation:

The article presents an analysis of quantitative and qualitative changes in the clinical forms of pulmonary tuberculosis (TB) following antimycobacterial therapy during the preoperative period, as well as the results of pathological examination of lung resections or whole-lung specimens obtained during pneumonectomies for pulmonary TB. The study included cases of pulmonary TB with surgical treatment for the period 2019-2023. The study group included 145 patients, most of whom initially received the main course of anti-TB therapy and subsequently additional surgical treatment, according to the form of TB lung damage. A morphological analysis of the activity of the specific inflammatory process was performed on the surgical material and the form of pulmonary TB was established or clarified. Modern trends in the medical pathomorphosis of pulmonary TB were elucidated in terms of changing the form-phase, as well as different activity of specific inflammation at the time of surgery. It was established that at the beginning of treatment, infiltrative and disseminated forms of pulmonary TB quantitatively prevailed in patients (in total, they amounted to 71.0%), which are a manifestation of the progression of the specific process. Whereas, by the time of surgery, tuberculomas were diagnosed in most cases (58.6%). The latter are considered a relatively favorable form, indicating a decrease in activity and limitation of the pathological process, although the morphological signs of the tuberculous inflammation activity in the affected area may vary. In addition, the number of cases of FCT has significantly increased (from 8.3 to 14.5%), which is associated with cases of ineffective chemotherapeutic treatment and the possible influence of other negative factors. Explanations of the findings from this comprehensive investigation are provided, along with recommendations for clinical practice.
