Annotation:

Chronic non-communicable diseases remain the leading cause of morbidity and mortality among older adults, highlighting the need to transform outpatient care based on integrated, patient-centered models. In Ukraine, these objectives are outlined in the National Health Care Strategy until 2030 and the Medical Guarantees Program. The aim of the study was to provide scientific justification and to develop a conceptual model for the organization of outpatient care for elderly people, taking into account WHO international recommendations, national regulatory documents, and current challenges facing the health care system. The research base was the outpatient medical care system of the State Institution of Science “Center of innovative healthcare technologies” State Administrative Department during 2015-2024. Bibliosemantic analysis, a system-structural approach, and the method of conceptual modeling were applied, which made it possible to assess morbidity, functional disorders, and organizational efficiency. The study was conducted in accordance with the provisions of the Declaration of Helsinki. A high prevalence of chronic conditions and functional impairments among older adults was established, particularly cardiovascular, respiratory, cognitive, and sensory disorders, as well as depressive states. The identified problems concern limited access to services, workforce shortages, insufficient technical resources, and the absence of effective patient routing. The proposed conceptual model integrates medical, rehabilitative, and social components, is based on a multidisciplinary approach, the use of digital technologies, and systematic health monitoring. Its implementation will contribute to maintaining the functional independence of elderly individuals, improving quality of life, and strengthening the resilience of the health care system.
