CLINICAL AND DIAGNOSTIC ASPECTS PLACENTA PREVIA IN PREGNANT WOMEN

A total of 115 pregnant women were assessed and divided into two groups. The retrospective group included 30 patients who underwent hysterectomy, while the prospective group comprised 85 patients managed with a uterus-preserving surgical approach based on a patented technique. All deliveries were performed via cesarean section. Diagnostic procedures were based on clinical history and examination, ultrasound (including Doppler imaging and MRI when indicated), and laboratory assessments such as coagulation profiles and hemostasiograms. The study identified statistically significant laboratory abnormalities in patients with central placenta previa: anemia, leukocytosis, neutrophilia, reduced lymphocyte and monocyte counts, and signs of hypercoagulability (shortened PT and APTT, elevated fibrinogen levels, and increased thrombin time). These findings point to an inflammatory-hypoxic syndrome and a heightened risk of thrombohemorrhagic complications. Statistical analysis revealed significant differences between the main cohort and the control group (healthy pregnant women). Moreover, strong associations were identified between central placenta previa and factors such as prior uterine surgeries, as well as maternal age and reproductive history. The findings emphasize the importance of early diagnosis and a personalized approach to managing pregnancies complicated by central placenta previa. The implementation of organ-preserving surgical techniques demonstrated promising results in reducing obstetric morbidity and improving maternal and neonatal outcomes. This study supports the need for a comprehensive clinical-diagnostic strategy and calls for the expansion of preventive and monitoring programs, particularly for women at high risk for placenta previa.
