Resections of large liver volumes are often complicated by the development of post-resection portal hypertension, which is complicated by bleeding from varicose veins of the oesophagus, stomach, rectum, ascites, splenomegaly, secondary hypersplenism, parenchymal jaundice, portosystemic encephalopathy, and multiple organ failure. The reproductive system organs are extremely vulnerable to various changes in haemodynamics, but to date, there is no complete understanding of the morphological and pathophysiological processes occurring in the testes under conditions of prolonged hypertension in the hepatic portal vein. When exposed to endogenous and exogenous negative factors, the haemomicrocirculatory bed plays an essential role in developing damage to organs and systems. The study aimed to investigate the peculiarities of the structural changes in the hemomicrocirculatory bed of the testes in the conditions of postresection portal hypertension using quantitative morphological methods. Morphological methods were used to study the vessels of the hemomicrocirculatory bed of the left and right testes of 62 laboratory mature male rats divided into three groups. Group 1 - 15 control animals, group 2 - 30 rats with postresection portal hypertension, and group 3 - 17 animals with postresection portal hypertension who developed multiple organ failure. The rats were euthanised by bleeding under thiopental anaesthesia 30 days after the start of the experiment. Postresection portal hypertension was modelled by removing the left and right lateral lobes of the liver (58.1% of its parenchyma). The morphometry of the hemomicrocirculatory vessels was performed on histological microsections of the testes. It has been established that prolonged postresection portal hypertension leads to narrowing of arterioles, precapillary arterioles, hemocapillaries, marked dilation of the testicular venules and venules, decreased microvascular density, venous blood flow, hypoxia, atrophy, dystrophy, necrosis of vascular endothelial cells, spermatogenic epithelial cells, stromal structures, infiltration and sclerosis. The degree of structural changes in the vessels of the hemomicrocirculatory bed is dominant in the left testis of experimental animals with a combination of postresection portal hypertension and multiple organ failure.
