РАК ГРУДНОЇ ЗАЛОЗИ У ЧОЛОВІКІВ: ФАКТОРИ РИЗИКУ, ДІАГНОСТИКА ТА ЛІКУВАННЯ

Анотація:

Метою огляду є вивчення захворюваності на рак грудної залози (РГЗ) у чоловіків, історії цієї патології, а також огляд факторів ризику можливостей діагностики та лікування, зокрема ад’ювантної ендокринної терапії. Були проаналізовані зарубіжні та вітчизняні джерел, а також результати декількох клінічних досліджень. Згідно з останніми клінічними рекомендаціями NCCN з лікування РМЗ, показанням до генетичного консультування є виявлений РГЗ у чоловіків. На 1-му етапі проводиться визначення мутацій в генах BRCA1 і BRCA2 методом полімеразної ланцюгової реакції. Діагностика РГЗ у чоловіків включає такі ж методи, що застосовуються у жінок. Враховуючи відносно невелику кількість випадків, рандомізовані контрольовані дослідження методів лікування РЗГ вкрай обмежені, і більшість рекомендацій ґрунтуються на дослідженнях раку молочної залози у жінок. У зв’язку з анатомічними особливостями органозберігаючі операції у чоловіків практично не виконуються (від 5 до 10%). Неоад’ювантна терапія проводиться за тими ж схемами, що і в жінок. Практично у всіх дослідженнях вказується на те, що в злоякісних пухлинах чоловічої молочної залози рівень рецепторів естрогенів у середньому вищий, ніж у жінок. Клінічно значущі рівні рецепторів естрогенів містяться більш ніж 85% пухлин молочних залоз у чоловіків. Тому адекватне лікування РГЗ неможливе без застосування ендокринної ад’ювантної терапії. Згідно останніх клінічних досліджень перевага надається блокаторам естрогених рецепторів та аналогам агоністів ГнРГ.
