Annotation:

An anomalous origin of the left coronary artery from the pulmonary artery is a rare congenital anomaly that can lead to ischaemia and myocardial infarction and is one of the causes of early infant mortality and sudden death in adults. From 2008 to 2024, 20 patients underwent surgical correction of anomalous coronary vessels from the pulmonary artery at the Amosov National Institute of Cardiovascular Surgery of the National Academy of Medical Sciences of Ukraine. Preoperative echocardiographic examination revealed a reduced left ventricular ejection fraction in 3 (15%) patients, initially reduced in 2 (10%) and preserved in 15 (75%) patients. Mild mitral insufficiency was observed in 5 (25%) patients, moderate - in 11 (55%), and severe - in 4 (20%) patients. In 15 (75%) patients, concomitant congenital heart disease was detected in combination with an anomalous left coronary artery from the pulmonary artery. Hospital mortality in the early postoperative period was 5% (1 patient). Postoperative echocardiographic examination showed recovery of LV EF in 4 patients out of 5 who had reduced or moderately reduced LV EF before surgery. In 6 (30%) patients, moderate mitral insufficiency persisted after surgery. In 4 (20%) patients, pulmonary artery stenosis developed in the long-term postoperative period. 1 (5%) patient underwent reoperation for pulmonary artery stenosis one year after ALCAPA correction. Patients with anomalous left coronary artery have a significantly increased risk of complications such as myocardial infarction, arrhythmias, and even death, so timely diagnosis and surgical correction of the defect can considerably reduce this risk and improve prognosis.
