Abstract. The aim of the study was to investigate the technological features and results of simultaneous laparoscopic hernioplasty of inguinal hernias (TAPP) and appendectomy for chronic appendicitis. The diagnosis of chronic appendicitis was based on clinical and anamnestic data (history of appendicular mass, recurrent pain in the right iliac region), and ultrasound and computed tomography. The analysis of baseline data, complications, duration of surgery and hospitalization in patients with simultaneous appendectomy and TAPP inguinal hernia repair in 25 patients compared with isolated TAPP in 28 patients. All patients underwent TAPP hernia repair according to the standard method: using three trocars (paraumbilical, right and left mesogastric). A 7.9 x 13.4 cm mesh graft was used for alloplasty, which was fixed with a herniostepler to the pubic bone, inguinal ligament and muscular layer of the anterior abdominal wall. In patients with simultaneous surgery, this was the first stage. In the second stage, a laparoscopic appendectomy was performed using an additional 5 mm trocar. In 15 cases, adhesioniosis was performed before appendectomy due to adhesions in the iliac fossa, and in three cases, moderate bleeding, suspended by electrocoagulation, was observed during adhesiolysis. There was no need for conversion or other features. Due to appendectomy, the duration of the operation increased by an average of 8 minutes. Any significant effect on the development of complications and duration of hospitalization of simultaneous appendectomy during inguinal hernia repair was not found. Thus, simultaneous appendectomy during TAPP inguinal hernia repair does not significantly affect the course of the operation and does not increase the risk of adverse postoperative events. In addition, direct laparoscopic imaging allows to assess the condition of the appendix and is an additional method of diagnosis in case of doubtful diagnosis of chronic appendicitis.
