Clinical-Psychological Aspects of Iron Deficiency in Six Years – Old Children, who are Preparing for the Systematic Training in School

The clinical features and emotionally-volitional functions were studied for children in age six years with the iron deficiency (ID), that will systematic study at school. Changes, that educed depended on the degree of iron deficiency. After the clinical and laboratory inspection in research 205 children were plugged in age 6±0,8 years, that was brought up in child’s preschool establishments. The basic group was entered by 155 children, from them 105 chil- dren with the latent iron deficiency (LID) and 50 children with iron deficiency anemia (IDA) I degree. The control group was made by 50 healthy children. For the study of characteristic clinical signs conducted: estimation of complaints (on the ball scale, where 0-no signs, 1 point – displays, 2 points – moderate displays, 3 points – considerable displays, are poorly expressed), ob- jective inspection after organs and systems, research of emotionally-volitional sphere after the level of anxiety (the test is «Anxiety» (R. Temml, М. Dorki, V. Amen)), after the features of behavior (the test-questionnaire of «Evaluation of level of anxiety and inclination of child is to the neurosis» (А. Zacharov)), after the level of development of arbitrary sphere (the test is «Graphic dictation»), ability to operate by rule (the test is «Labyrinth»), ability to operate accord- ing to (the test is «House») sample. For an exposure statistically of meaningful differences used the non-parametric exact criterion of Fisher exact two-tailed for comparison of frequency sizes and criterion β2 of four-course table of conjugating. For authenticity of results accepted differences at р<0,05. Researches educed that to put all 6-years-old age from LID had clinical signs of syndrome sideropenia. Most often there were symptoms of – blue sclera – 38%, dryness of skin – 37,1%, fragility and stratification of nails – 26,7%, distortion of taste – 24,8% and other. The degree of severity of symptoms in almost half of the children had moderate. Among the most common signs of anemic syndrome marked pallor of the skin and mucous membranes – 52,4%, loss of appetite – 41,9%, fatigue – 6,6%. Severity displays data was weak in most children. Children with IDA grade I had more severe symptoms sideropenia and they met often. Marked increase in the severity of symp- toms and more anemic symptoms: pale skin and mucous membranes – 84%, systolic murmur on auscultation at the apex of the heart or at Botkin – 74%, loss of appetite -52%, fatigue – 16%, irritability – 16%, lethargy, 14%, headache 4%. All the above symptoms ID combined together in different ways. Comparison of the frequency and severity of symptoms between children with ILD and IDA showed clinical signs of dependence on the degree of the ID. Most children with ID had an increased level of anxiety, behavior changes and delayed formation of the regula- tion of voluntary sector. Children with ID clot anxiety after something fear of aggression from others, they feared punishment for his «bad» behavior, change habitual environment, peer rejection, separation from the mother, loneliness, reprimands from parents. Most of the children were expecting something unpleasant, uncertain. Assessment of behavioral responses showed that children with high and above average levels of anxiety LID and IDA have similarities in be- havior: easily upset; many experienced, often resented; fear of new situations; have been extremely volatile in the mood too excited, often distracted, were restless; quickly tired. Most children with ID age of six who are preparing for entry into school was average and poor regulation of voluntary activity, in contrast to healthy children. Identified abnormalities were higher in children with IDA than LID. Given that the ability to carefully and accurately perform consistent adult guidance, to act according to the rules and the model is a prerequisite for learning activities sug- gests that these children may later have difficulty in learning.
